A panel of distinguished participants in the food and agricultural system offered perspectives on the impacts and how we can learn from them. \nInvited perspectives include: \n\nA.G. DESCRIPTION:The FDA and the University of Texas Medical Branch\, Galveston National Laboratory (UTMB) will host this year’s annual training course on how to meet GLP requirements in BSL-4 facilities from April 20-24\, 2020 at the National Institutes of Health (NIH) in Bethesda\, Maryland. \nScientists working to develop Medical Countermeasures (MCMs) against several high-priority biological threat agents are at high risk of aerosol-transmitted laboratory infection and life-threatening disease\, as there are no vaccines or treatments for these agents. Accordingly\, much of the work to develop MCMs for these high-priority agents must be done in high and maximum biosafety level (BSL)-4 laboratories to prevent the agents from being released into the environment\, and to provide maximum safety for the scientists. \nThe unique challenges of BSL-4\nWhile necessary to ensure lab worker safety and prevent accidental release of dangerous pathogens\, BSL-4 environments pose unique challenges to developing MCMs. For example\, data generated to support a regulatory submission to FDA must be collected in accordance with quality systems\, like good laboratory practice (GLP) regulations\, to be useful and reliable\, and to facilitate FDA review to ensure safety and efficacy. \nHowever\, meeting data quality and integrity standards like GLP in a BSL-4 environment is challenging. Current practice is often inconsistent among laboratories due to technological infrastructure\, available internal standard operating procedures\, level of regulatory awareness\, and individual breadth of experience and training. And completing even routine lab tasks in a protective BSL-4 suit can be difficult—imagine\, for example\, trying to pick up a small item\, like a toothpick\, while wearing two pairs of thick gloves. \nTo help address the challenges associated with ensuring data quality and integrity in regulated studies conducted in BSL-4 laboratories to support MCM development\, FDA and the University of Texas Medical Branch\, Galveston National Laboratory (UTMB) collaborate to provide an annual training course on how to meet GLP requirements in BSL-4 facilities (“data quality course”). \n\nThis popular course offers a unique opportunity for the regulatory and scientific communities to discuss complex issues in an interactive environment and identify and share best practices for ensuring data quality and integrity in BSL-4 facilities. \n\nIn addition to UTMB and FDA\, other federal partners from the National Interagency Confederation for Biological Research (NICBR) provide support and resources for this course. These partners include: \n\nFrom the National Institutes of Health\n\nOffice of the Director\nNational Institute of Allergy and Infectious Diseases (NIAID)\nNational Cancer Institute (NCI)\n\n\nFrom the Department of Defense\n\nU.S. Army Medical Research and Materiel Command (USAMRMC)\n\n\n\n\nFrom the Department of Homeland Security\n\nNational Biodefense Analysis and Countermeasures Center (NBACC)\n\n\n\nHistory and new additions\nSince the data quality course began in 2013\, it has quickly reached capacity each year. In its first five years\, 269 participants completed the course. \nIn September 2017\, the FDA Medical Countermeasures Initiative (MCMi) renewed the grant  to continue the data quality course for an additional five years\, from 2018-2022. UTMB and FDA will also expand on-demand online offerings\, adding data quality-related good clinical practices (GCP) training and other courses to meet the needs of researchers responding to emerging infectious disease outbreaks. \nCourse description\nThe week-long\, data quality course is designed to help researchers who conduct studies intended to support approval under the Animal Rule\, which may be used to grant marketing approval of certain products when human challenge studies would not be ethical or feasible. \nThe course includes expert lectures and hands-on laboratory activities conducted in BSL-2 and BSL-4 training laboratories to emphasize the differences between biosafety levels\, and the complexity of conducting laboratory activities in a BSL-4 environment. An online training module on GLP regulations (21 CFR Part 58) is included as a course pre-requisite. He continues to work with HHS and other agencies on issues of hospital surge capacity planning\, adjusted standards of care in disasters\, and preparedness for nuclear terrorism.\n\n  \n\nRebecca E. Gee\, MD\, is the Secretary of the Louisiana Department of Health\, the state’s largest agency with a budget of $14 billion\, and Gratis Assistant Professor at the LSU Schools of Public Health and Medicine. Her oversight responsibilities include public health and other direct service programs for citizens in needs such as behavioral health\, developmental disabilities\, aging and adult services\, emergency preparedness ad the Medicaid program. Her Medicaid expansion work resulted in an additiona 500\,000+ Louisiana residents being newly insured and the launch of a dashboard to measure access to health care services that has become a national model used in other states. She is an OB/GYN\, trained policy expert and previously served as the director of the Birth Outcomes Initiative\, where she led the charge to decrease infant mortality and prematurity statewide — an effort that in part led to a 25% reduction in infant mortality\, an 85% drop in elective deliveries before 39 weeks and a 10% drop in NICU admissions.\n\n  \n\nJeffrey Kahn\, PhD\, MD\, is the is the Andreas C. Dracopoulos Director of the Johns Hopkins Berman Institute of Bioethics and the inaugural Robert Henry Levi and Ryda Hecht Levi Professor of Bioethics and Public Policy. He is also Professor in the Department of Health Policy and Management of the Johns Hopkins Bloomberg School of Public Health. He works in a variety of areas of bioethics\, exploring the intersection of ethics and health/science policy. Kahn has served on numerous state and federal advisory panels and is currently chair of National Academies of Sciences\, Engineering\, and Medicine’s Board on Health Sciences Policy\, and has previously chaired its committee on the Use of Chimpanzees in Biomedical and Behavioral Research; the committee on Ethics Principles and Guidelines for Health Standards for Long Duration and Exploration Spaceflights; and a committee on the Ethical\, Social\, and Policy Considerations of Mitochondrial Replacement Techniques. He also formerly served as a member of the National Institutes of Health Recombinant DNA Advisory Committee. In addition to committee leadership and membership\, he is an elected member of the National Academy of Medicine and an elected Fellow of The Hastings Center. He was also the founding president of the Association of Bioethics Program Directors. He is a co-principal investigator with Berman Institute faculty member Gail Geller\, ScD\, MHS\, on GUIDE: Genomic Uses in Infectious Disease and Epidemics\, an NIH-funded project to study the largely unexplored ethical\, legal and social implications of genomics as applied to infectious disease.\n\n  \n\nLawrence “Larry” O. Gostin\, JD\, directs the O’Neill Institute for National and Global Health Law and is the founding O’Neil Chair in Global Health Law at Georgetown University. He served as associate dean for research at Georgetown Law from 2004-2008 and is professor of medicine at Georgetown University and professor of public health at Johns Hopkins University. He is director of the World Health Organization Collaborating Center on National and Global Health Law. Among his appointed posts at WHO\, he served on the Director-General’s Advisory Committee on Reforming the World Health Organization\, is on the International Health Regulations Roster of Experts and has served on many WHO expert advisory committees including on the Pandemic Influenza Preparedness Framework and on the WHO/Global Fund Blue Ribbon Expert Panel: The Equitable Access Initiative to develop a global health equity framework. He co-chairs the Lancet Commission on Global Health Law. He served on two global commissions to report on the lessons learned from the 2015 West African Ebola epidemic and was senior advisor to the United Nations Secretary General’s post-Ebola Commission. In the aftermath of this unprecedented pandemic\, the United States will need to reconsider its approach to biosurveillance\, public health emergency preparedness\, intergovernmental coordination\, and the development of countermeasures in advance of the next health crisis. In the past\, such efforts have been hindered by partisan polarization between Republicans and Democrats\, even as some crucial public health emergency policy was only made possible through bipartisan cooperation. While considering lessons from the past\, this discussion will address bipartisan efforts to move forward on approaches to protect the American people in the future\, such as the implementation of the National Biodefense Strategy. \nDr. Nathan Myers is an associate professor of political science and public administration at Indiana State University. His research interests include public health policy\, U.S. executive branch politics and administration\, emergency planning and preparedness\, and regulation of biotechnology. DESCRIPTION:The Role of Digital Health Technologies in Drug Development: A Workshop will be webcast on March 24\, 2020. \nAs digital health technologies (e.g. smartphone apps\, wearables\, and mobile-device-based tools) – hardware and software – become increasingly available to consumers\, providers\, and researchers\, there are new opportunities to address critical barriers or “pain points\,” better connect patients and health care providers\, and incorporate patient input throughout the drug research and development (R&D) life cycle. However\, a lack of clarity when it comes to the evaluation\, selection\, and implementation of digital health technologies has impeded the practical application of these innovations. Lessons learned from the direct-to-consumer genomics industry\, which has been increasingly moving into the clinical health space over the last 10 years\, may help inform the integration of digital health technologies into drug R&D. \nThis workshop will provide a venue to discuss challenges and opportunities in digital health technologies for improving the probability of success in drug development and enabling precision medicine. Workshop participants may consider key components for an evidence-based framework for applying digital health technologies towards drug research and development. \nThe Forum on Drug Discovery\, Development\, and Translation and the Roundtable on Genomics and Precision Health will host a public workshop on March 24\, 2020 which will feature invited presentations and discussions to: \nDiscuss challenges/questions that digital health technologies may be uniquely suited to address and opportunities for digital health technologies to enable better patient care and more efficient clinical trials; \nConsider strategies for evaluating and selecting digital health technologies that are fit-for-purpose in drug development (e.g. establishing appropriate evidentiary criteria); \nDiscuss privacy\, ethical\, and regulatory issues related to the use of digital health technologies; \nExamine currently validated and/or FDA-approved drug development applications for digital health technologies; \nConsider opportunities to enable the practical application of digital health technologies for improving drug development (e.g. sharing best practices for the validation and use of digital health technologies\, harmonizing guidelines across sectors).
